This waiver is effective for all participants listed below for any and all Fly Dance Competition events for the 2025 calendar year.

I understand the legal consequences of signing this document, including (a)releasing the FLY Dance Competition (FDC) from all liability on my behalf & the participant’s behalf, (b) waiving my and the participant’s right to sue FDC, (c) and assuming all risks of dancers participation at FDC
